XBox "Names"

I remember back when I was playing Halo over LAN with friends they'd have different names. What deteremined this originally? Did every XBox have a unique name or did they repeat? Any other games show this name aside from Halo?
I know the common one was DOA3 to change the name, I've also heard MotoGP can as well.
